Application of Balloon Catheter in Vascular Calcification

With the aging of the population and the increase of diabetes mellitus and chronic kidney disease, more and more coronary artery calcification lesions appear in patients with coronary heart disease,ptca balloon dilatation catheter which brings great challenges to interventional therapy, and is also an important reason for the failure and complications of interventional therapy, and it is also the nightmare of interventional doctors. Therefore, in recent years, new instruments and interventional techniques for calcified lesions have emerged, which provides a powerful tool for interventional treatment of such lesions.
Although the rotary grinding technique plays an important role in the interventional treatment of severe calcification, especially annular calcification, it needs special equipment and is complicated. At present, it is only routinely carried out in large heart centers in China, and most centers still use traditional balloon technique to pretreat such lesions. In addition to a small number of circular intimal surface calcification, the majority of adventitia calcification and non-circular calcification can be successfully implemented by using special balloon technology.
Before the decision of interventional therapy for calcified lesions, the degree of calcification should be carefully evaluated, and if necessary, intravascular ultrasound should be used. In the pre-expansion of calcified lesions, the balloon with good trafficability and strong pushing force of push rod should be selected.
In the selection of balloon, the balloon with smaller diameter, such as 1.5mm, should be selected first. If the balloon passes smoothly and the expansion effect is good, then use a longer balloon of 2.5mm for expansion. The expansion pressure is generally not more than 14atm, and the maximum is not more than 18atm. In the process of expansion, the pressure should be slowly pressurized. If the pre-expansion effect is not good, double wire balloon, high pressure resistant nipple balloon and cutting balloon can be used to further improve the lesion. Before stenting, the used balloon can pass through calcified lesions. If the stent passes smoothly, the stent can be implanted; if the stent still cannot pass, the Guidezilla extended catheter can be used to deliver the stent. The stent should be expanded and released under high pressure, and then fully expanded by non-compliant balloon with good performance after release, so as to ensure the stent fully adheres to the wall.
