On October 2, 2021, Kossel Medtech independently developed innovative medical device — the first domestic umbrella-shaped long-acting vena cava filter Octoparms®, completed the first implantation application in Nanping City in Nanping First Hospital. Director Gao led the team of vascular Surgery department to perform the operation. Kossel is a manufacture of PTCA BALLOON DILATATION CATHETER and PTA BALLOON CATHETER. Their annual capacity will reach to 800,000pcs next year
1.Case Description
Patient information: Female, 68 years old
Medical history: She was admitted to the pain department for 7 days due to low back pain caused by trauma. Vertebroplasty surgery was planned to be performed. Before the surgery, color ultrasound found complete thrombosis in the upper segment of the superficial branch of the right external iliac vein, common femoral vein, superficial femoral vein, popliteal vein and posterior tibial vein. Please consult my department before surgery.
Cervical and lumbar MR showed:
①T6 and L2 vertebral compression fractures with bone marrow edema were considered, T3 and T10 old compression fractures were considered.
②Cervicothoracolumbar degenerative osteoarthropathy with a small amount of endplate inflammation at some vertebral margins is shown.
Previous history: Previous history of lumbar fractures.
2.Preoperative Diagnosis
There was obvious swelling of the right lower limb, with mild tenderness, no local skin temperature rise, no skin ulcer, and no obvious weakening of dorsalis pedis artery pulse. Pericardial super deep veins of both lower limbs (2021-09-30) : complete thrombus formed in the upper segment of the superficial branch of the right external iliac vein, common femoral vein, superficial femoral vein, popliteal vein and posterior tibial vein.
3.Operation Plan
Vertebroplasty was required due to a lumbar fracture. After preoperative notification of surgical risks and considering the long time window, it is recommended to insert a Kossel filter.
4.Operation Note
The patient was supine on the operating table, the skin of the surgical field was routinely disinfected, and sterile towel sheets were spread. After successful local anesthesia with lidocaine, sedlinger technique was used to puncture the left femoral vein, and porcine tail catheter was sent to the left iliac vein for inferior vena cava angiography. The inferior vena cava was clear and the position of the opening of both renal veins was determined. The opening of the right renal vein was about the level of the upper edge of L1. An Octoparms® vena cava filter was placed, open and in a satisfactory position. The operation went well and was safely returned to the ward.
5.Conclusion Postoperative
The Octoparms® vena cava filter is a stage-mounted design with four pairs of balancing arms, which is neutral, stable, full when opened and easy to operate, and has the potential to benefit a wider range of patients with thrombosis.
